ASM® Modular femoral stem case


Basic information of the patient: male, 40 years old, from Leping, Jiangxi Province, was admitted to hospital on March 20, 2019 due to swelling and pain of left hip and dysfunction for more than 30 years.

Case characteristics:

1. The patient is a middle-aged male with chronic onset.

2. The main symptoms were swelling and pain of the left hip and dysfunction for more than 30 years. The patient was diagnosed and treated in the First Affiliated Hospital of Nanchang University because there was no obvious inducement more than 30 years ago and the activity of swelling and pain of the left hip was limited. Anti tuberculosis treatment was given to the patient. The details were unknown. The patient felt that the activity of the left hip was limited gradually, and the symptoms were obvious when standing and walking, and he could not squat, In adults, the patient felt left lower limb shortening, walking lameness and left hip joint dysfunction.

3. Pelvic plain film examination in outpatient department showed tuberculosis of left hip joint and necrosis of femoral head.

4. Pelvic CT examination showed that the left femoral head was avascular necrosis and degenerative changes, and the hip joint was subluxated, so the left hip joint tuberculosis was admitted.

5. Special situation: there is physiological curvature of lumbar vertebra, no lateral process deformity, left lower limb atrophy, slightly shorter than right, no swelling of left hip joint, deep tenderness of hip joint, pain of longitudinal percussion, and basic loss of left hip joint activity. Four word test: left side positive, right side negative.

Preoperative preparation:

osteotomy: L-shaped osteotomy close to the greater trochanter

Selection and placement of acetabular prosthesis: try to fix the prosthesis acetabulum in the true acetabulum position. In special cases, fix the prosthesis acetabulum in the false acetabulum to solve the problem of femoral head reduction or limb length. The anteversion of acetabulum should be adjusted during operation.
