Revision Hip Joint Replacement in Elderly Patients
Patient information
The male was 72 years old.
Chief complaint
The left hip pain activity was disadvantageous for 1 day due to fall injury.
History of Present Illness
One day ago, the patient suffered from left hip pain due to accidental fall injury. He could not walk, and there was no empty glenoid or elastic fixation. He had a history of left total hip arthroplasty.
Physical examination
Old surgical scar can be found in the left hip, and the healing is good. Left hip swelling, tenderness, limited movement, greater trochanter percussion pain, lower limb longitudinal axis percussion pain positive.
Before the operation
In the operation
After the operation
The patient had a history of hypertension and a history of left total hip arthroplasty ten years ago. The fracture around the prosthesis caused by fall injury is accompanied by prosthesis loosening. Combined with auxiliary examination, it has the indication of joint revision operation, but considering the patient's previous history of artificial joint replacement operation, and the patient's age, the operation risk is very high. During the operation, the soft tissue adhesion is obvious, the upper femur long oblique fracture, and the acetabular prosthesis wear is obvious. Therefore, the prosthesis should be replaced, local compression and bone grafting should be carried out, and the upper femur fracture end should be bound with steel wire to reduce the stripping To reduce iatrogenic injury and choose lengthened stem prosthesis, the patients recovered well after operation.
