JUST Lateral Decubitus DAA Minimally Invasive Total Hip Replacement Surgery

Recently, Dr. Zhang Chao and his team from the Orthopedics Department of Yunnan Provincial First People's Hospital performed a lateral decubitus DAA (Direct Anterior Approach) minimally invasive total hip replacement on a woman in her sixties suffering from rheumatoid hip arthritis. Despite receiving standard anti-rheumatic treatment, her condition had worsened, causing severe hip pain and difficulty walking, even with assistance.

Condition Overview and Treatment Outcome

After admission, under the guidance of Professor Meng Zengdong, Dr. Zhang Chao's team considered the patient's age and health, recommending the lateral decubitus DAA minimally invasive total hip replacement to avoid prolonged bed rest. They explained that this approach offers minimal soft tissue separation, reduced muscle damage, and faster recovery. After discussing it with her family, the patient agreed to the procedure.

With careful preparation and coordination with the anesthesia and nursing teams, the surgery was successfully performed under general anesthesia. No urinary catheter, drainage tube, or blood transfusion was needed. Six hours post-surgery, the patient's vital signs were stable, and she experienced no pain at the incision site. Under guidance, she began walking exercises, much to the surprise of her and her family.

Preoperative

Preoperative X-rays showed severe right-sided rheumatoid hip arthritis with femoral head necrosis and joint surface collapse.

Postoperative

Postoperative X-rays showed that the prosthesis was well-positioned.

· Ti6Al4V material with Ti+HA dual coating (225μm pure titanium coating + 70μm  hydroxyapatite coating per side) ensures long-term bone ingrowth

· Proximal rectangular cross-section resists axial and rotational stress, ensuring  initial stability

· Lateral shoulder cutting design makes implantation smoother

· Proximal stepped design optimizes stress distribution, converting ring shear stress  to axial compressive stress, improving axial stability

· Distal vertical and horizontal grooves provide rotational and axial stability

· Offset and stem length design better fit Asian bone characteristics, accurately  restoring the biomechanics of the lower limb

· Extensive size range to address a wide range of patients

· Ti+HA double-layered vacuum plasma coating of 0.375mm provides  excellent initial press-fitting and 15 years of proven clinical results

· One cup can be coupled with different liner options

· Mirror-polished inside to effectively reduce wear

· A high proportion of dome support maximizes liner-cup contact, reducing  micromotion and wear
